Essential Job Functions/Responsibilities

ย ย ย ย ย ย ย ย  Provides direct patient care as defined in Oregon's Nurse Practice Act

ย ย ย ย ย ย ย ย  Implements plan of care

ย ย ย ย ย ย ย ย  Provides accurate and timely documentation consistent with the plan of care

ย ย ย ย ย ย ย ย  Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care

ย ย ย ย ย ย ย ย  Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse

Qualifications

ย ย ย ย ย ย ย ย  Graduate of an accredited practical nurse or vocational nursing program

ย ย ย ย ย ย ย ย  One year nursing experience preferred

ย ย ย ย ย ย ย ย  Currently licensed as a licensed registered nurse in the state

ย ย ย ย ย ย ย ย  Complies with accepted professional standards and practice

ย ย ย ย ย ย ย ย  Demonstrates good verbal and written communication, and organization skills

ย ย ย ย ย ย ย ย  Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order

ย ย ย ย ย ย ย ย  Possesses and maintains current CPR/BLS certification from AHA
