• Actively engage with participants to identify their needs and develop tailored strategies that minimize reliance on crisis services, thereby enhancing their independence and stability.
• Document, coordinate, and maintain crisis alerts for clients, ensuring timely collaboration with county crisis service teams.
• Oversee the transition of patients between various care settings, including hospitalizations, ensuring that discharges are managed smoothly and effectively in collaboration with hospital staff.
• Provide ongoing therapeutic support through regular virtual counseling sessions, utilizing evidence-based practices to address a variety of mental health challenges.
• Conduct thorough assessments and create individualized treatment plans that reflect clients’ goals and unique circumstances.
• Communicate sensitive information effectively, employing culturally aware and client-appropriate language that considers each client’s unique background.

Our clients represent a diverse population encompassing various cultural, linguistic, and psychological backgrounds. As a Telehealth Professional Counselor, you will work with individuals facing an array of psychiatric, emotional, and social challenges. • A Master's degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.
• An active and unrestricted professional license (LMFT, LPC, or LCSW) in the state of Oregon.
• A proven ability to rapidly establish rapport and trust with clients from diverse backgrounds.
• Strong communication skills that demonstrate sensitivity to the needs of clients and their families, as well as to teammates and other healthcare professionals.

• Experience working in telehealth or virtual care settings is preferred but not mandatory.
• Familiarity with crisis intervention strategies and experience in managing complex mental health situations.
