Director of Rehabilitation (DOR) - Full-Time | Sheboygan, WI

Setting: Skilled Nursing Facility (SNF)

Full-Time Rehabilitation Leadership Opportunity

Continuum Therapy Partners is seeking a Director of Rehabilitation (DOR) for a full-time leadership position at Sheboygan Health Services in Sheboygan, Wisconsin. This is an excellent opportunity for an experienced rehabilitation professional who is passionate about clinical excellence, team leadership, and improving patient outcomes in a skilled nursing and post-acute care environment.

We are currently considering licensed Physical Therapists (PTs), Certified Occupational Therapy Assistants (COTAs), and Speech-Language Pathologists (SLPs) for this Director of Rehabilitation role.

Director of Rehabilitation (DOR) Responsibilities
  • Oversee the daily operations of the rehabilitation department.
  • Lead and support Physical Therapy, Occupational Therapy, and Speech Therapy staff.
  • Ensure compliance with Medicare, state, federal, and company regulations.
  • Monitor productivity, scheduling, documentation, and clinical performance metrics.
  • Promote high-quality patient care and positive rehabilitation outcomes.
  • Collaborate with physicians, nursing leadership, social services, and interdisciplinary team members.
  • Mentor therapists and therapy assistants through coaching, training, and professional development.
  • Participate in care planning, quality improvement initiatives, and facility leadership activities.
Qualifications
  • Current Wisconsin license as a Physical Therapist (PT), Certified Occupational Therapy Assistant (COTA), or Speech-Language Pathologist (SLP) (ASHA certification required for SLP)
  • Previous rehabilitation leadership, management, or supervisory experience preferred.
  • Skilled nursing facility (SNF), long-term care, rehabilitation, or post-acute care experience strongly preferred.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Commitment to patient-centered care and clinical excellence.
