Registered Nurse – RN Case Manager (Direct Hire)
Shift: 8-hour morning–afternoon
Location is North of Atlanta 
Pay is between $85000-95000 based on experience

Overview
Prestige Staffing is seeking an RN Case Manager to provide comprehensive patient assessment, care coordination, discharge planning, and length-of-stay management. This role partners closely with physicians, patients/families, nursing, utilization review, and the interdisciplinary team to support safe transitions of care and quality, cost-effective outcomes across the care continuum. Position may support patients across neonate through geriatric populations and provides cross-coverage as needed.

Key Responsibilities

  • Monitor patients on assigned units to ensure appropriate resource utilization and interventions; manage length of stay based on DRG/admitting diagnosis
  • Communicate expected length of stay and discharge goals with physicians, patients/families, and the care team
  • Coordinate and facilitate sequencing of care interventions; identify delays and escalate unresolved issues to leadership
  • Assess, coordinate, and facilitate discharge plans; arrange post-acute services prior to discharge; reassess throughout stay to ensure timely, safe transition
  • Provide patients/families with information on plan of care, discharge needs, and potential financial responsibility for services
  • Maintain knowledge of reimbursement methodologies and coverage guidelines; communicate as needed to support covered services
  • Partner daily with Utilization Review to ensure appropriate patient status/level of care; facilitate changes with providers and nursing
  • Serve as a liaison/leader for discharge planning, community resources/referrals, social service needs, and financial considerations
  • Ensure appropriate handoffs during unit-to-unit transitions
  • Maintain knowledge of community resources, payer requirements (PPO/HMO), and governmental regulations; complete required documentation
  • Adhere to all regulatory/accreditation requirements; demonstrate urgency and strong stewardship mindset
  • Work scheduled shifts, including weekend rotation and remote coverage

Minimum Qualifications (Required)

  • Active RN license in Georgia
  • Associate degree; graduate of an accredited school of nursing (Bachelor’s degree preferred)
  • CCM (Certified Case Manager) or ACM certification preferred
  • 3–5 years of experience in direct patient care and/or case management
  • Financial and discharge planning experience
  • Working knowledge of state and federal regulations
  • Strong observation, analytical thinking, problem-solving, self-direction, and oral/written communication skills
  • Professional, team-oriented interpersonal skills; positive approach
  • Aptitude in critical care, cardiac, medical, and/or surgical nursing
