Schedule Details/Additional Information:

Full Time Home Health Float Pool position based in WI supporting the southern patient service markets. Areas of work based on assignment. Typical schedule is Monday through Friday 8:00am to 4:30pm with some variability based on departmental needs. Every 4th weekend rotation required.

Pay Range:

$35.50 - $53.25

Major Responsibilities

  • Complete scheduled patient visits and collaborate with the interdisciplinary team.
  • Conduct comprehensive patient assessments and update plans of care as needed.
  • Communicate treatment changes with physicians and the primary registered nurse.
  • Monitor patientresponseto care and revise interventions accordingly.
  • Coordinate schedulingandcase conferenceswith primarynursewhenappropriate.
  • Assistswith the discharge planning process according to agency standards andutilizesinteragency/system and communityresources to ensure continuity of care after discharge.
  • Communicates problems to physicians and primary Registered Nurse, and clearlystatesplans,actionsand goals so thatchanges are reflected as they occur.
  • Maintainaccuratedocumentation in compliance with all regulations.
  • Follow agency policies and safely perform required patient care activities.
  • Provide age-appropriate care based on patient needs and developmental stage.

Minimum Job Requirements

Education

  • Associate's degree in nursing.

Certification / Registration / License

  • Active RN license in state of practice.
  • Basic Life Support (BLS) - American Heart Association
  • Valid driver's license and reliable transportation.

Experience

  • 3-4 years clinical experience.
  • 1-2 years in home health.

Knowledge / Skills / Abilities

  • Ability to work independently and make soundclinicaldecisions
  • Excellent communication (written and verbal) and interpersonal skills.
  • Ability to develop rapport andmaintainpositive, professional relationships with a variety of patients,staffand physicians.
  • Cultural competence and respect for patient diversity.
  • Good organization,prioritizationandproblem-solvingskills.

Physical Requirements and Working Conditions

  • Ability to multi-task with frequent interruptions.
  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be ableto lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies
