• Deliver comprehensive mental health services tailored to individual client needs including supportive counseling, crisis intervention, and grief counseling, addressing specific challenges such as coping with illness and treatment planning.
• Independently manage a diverse caseload that varies in clinical acuity, ensuring high-quality care and appropriate interventions for each patient.
• Conduct thorough psychosocial assessments upon patient admission and periodically thereafter, identifying barriers to care and recommending appropriate resources.
• Engage in case finding to identify patients and families with potential psychosocial needs and collaborate with healthcare professionals during team conferences and interdisciplinary rounds.
• Provide psychoeducation to patients and families about diagnosis, treatment plans, and transitioning from youth to adult services, empowering them to make informed decisions about their care.
• Develop and implement patient-centered psychosocial treatment plans in collaboration with interdisciplinary care teams, documenting all progress and interventions appropriately.
• Supervise MSWs and student interns, offering clinical guidance and contributing to their professional development.
• Participate in quality assurance programs and community improvement initiatives focusing on enhancing social work services.
• Maintain accurate and timely electronic medical records, adherence to policies, and prepare both written and oral reports regarding patient status.

You will engage with a varied client base, including adult, geriatric, pediatric, and adolescent populations. Our clients often face a myriad of environmental and psychological challenges related to mental illness and life transitions. As a Licensed Mental Health Counselor, you will play a crucial role in helping them navigate these complexities and improve their quality of life.

• A valid state license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
• A master’s degree in behavioral sciences or a related field from an accredited institution.
• Proficient in effective assessment, interviewing, and counseling techniques.
• Ability to draft detailed case narratives and maintain precise records.
