Job description

MDS Coordinator (RN) - Clinical Care Planning & Assessments

Locations (On Site): This position will support Cedar Falls (Deery Suites, Martin Suites), Grundy Center (Creekside), and Madrid (Madrid Home).

Western Home Communities is seeking a Registered Nurse (RN) MDS Coordinator to lead the MDS/RAI process, care planning, and resident assessments in a dynamic long-term care environment. This role is ideal for an RN who enjoys clinical coordination, regulatory compliance, interdisciplinary teamwork, and resident-centered care planning.

You will play a key role in ensuring accurate MDS submissions, comprehensive care plans, quality outcomes, and survey readiness while collaborating closely with nursing leadership, CNAs, PCP's, and families.

What you'll do as an MDS Coordinator:

  • Manage MDS/RAI assessments and electronic submissions
  • Monitor that individualized resident care plans are developed-notify DON of deficiencies
  • Code MDS according to assessments completed by clinical staff
  • Monitor that psychotropic medications are addressed, associated AIMS assessments are conducted, appropriate diagnoses are on the record, notify DON of deficiencies.
  • Ensure regulatory compliance, documentation accuracy, and survey readiness - making recommendations where necessary, and training employees on requirements
  • Support admissions, chart audits, and resident status changes
  • Educate and collaborate with nursing staff, CNAs, PCP's, and families
  • Attend (remotely) morning meetings for each location.

What you'll need as an MDS Coordinator:

  • (Required) Active Iowa RN license
  • (Required) Graduate of an accredited RN program
  • (Required) Ability to travel to WHS locations- must have valid Iowa driver's license and reliable vehicle
  • (Required) MDS or OASIS experience (long-term care experience strongly valued)
  • (Preferred) Experience in PCC
  • Strong knowledge of clinical documentation, care planning, and regulatory compliance
  • Excellent communication and organizational skills
  • Ability to work independently in a fast-paced LTC setting
  • Current in BLS CPR Certification
