Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of recent home health experience and HCHB EMR experience.

Details
  • Home health setting covering Snohomish and Skagit County (Mount Vernon) and Island territory
  • Territory extends up to a 60-mile radius from the hub office
  • Day shift schedule, 8-hour shifts, Monday through Friday, 8:00 AM to 5:00 PM
  • No weekend or on-call requirements
  • Patient population primarily geriatric
  • Average caseload of 5-7 patients per day with 6 visits daily
  • Productivity requirement of a minimum of 30 points weekly
  • Uses HCHB EMR system
  • Requires in-office visits weekly or biweekly for team meetings and supply/admission packet pickup
Requirements
  • Minimum 1 year of recent home health physical therapy experience required
  • BLS certification required
  • OASIS experience required
  • HCHB EMR experience required
  • Experience with geriatric patient population
  • Must be flexible with territory assignments
