As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes. You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support. KEY RESPONSIBILITIES • Provide high-quality acute care services to patients enrolled in the Hospital at Home program. • Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs. • Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment. • Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home. • Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes. Job Summary As a Registered Nurse, will oversee patient care provided by the healthcare team using a professional practice model. Your responsibilities include patient assessment, recognizing health issues, creating care plans, and assessing patient responses. What You'll Do You, as an RN, will conduct detailed clinical assessments, prioritize patient and family needs, create, implement, and evaluate care plans. Adjust plans to reach desired outcomes. Know nursing laws, rules, standards, and guidelines in your area. Understand hospital procedures and rules. Proficiency in medical terms, focusing on health, disease prevention, management, common meds, and their side effects. Respect for medical and professional nursing ethics and patient privacy rights.
