Job description

PRN Registered Nurse – Home Care Assessment

Home in the Heart Healthcare Agency, LLC is seeking a compassionate and dependable PRN Registered Nurse (RN) to complete an in-home nursing assessment/oversight visit for one of our home care clients.

This is a very part-time, as-needed position, currently requiring approximately one client visit per month. The RN will travel to the client’s home to complete the required assessment and nursing oversight documentation.

Responsibilities

  • Complete an in-home RN assessment/oversight visit approximately once per month.
  • Assess the client’s current condition, functional abilities, safety needs, and home care needs.
  • Review the client’s activities of daily living (ADLs) and level of assistance required.
  • Review and update the client’s plan of care as appropriate.
  • Monitor the effectiveness and appropriateness of homemaker/home health aide services.
  • Identify and document any changes in condition, concerns, or additional care needs.
  • Communicate relevant findings or recommendations to Home in the Heart Healthcare Agency management.
  • Complete all required nursing documentation accurately and timely.
  • Notify the agency of significant changes or concerns requiring follow-up.

Schedule

PRN / Part-Time – As Needed

Currently, this position involves approximately one assessment visit per month for one client. Additional assessment opportunities may become available as the agency grows.

  • Strong assessment, documentation, and communication skills
  • Dependable and able to work independently
