This is a full‑time role for a Home Health and Hospice RN. We will train in‑house through lots of shadowing with another RN. As a Home Health RN, you will play a critical role in providing excellent patient care in the comfort of their own homes.

Responsibilities
  • Assessing patient needs and creating care plans
  • Administering medications and monitoring vital signs
  • Providing wound care
  • Collaborating with a multidisciplinary team to ensure holistic well‑being and help patients achieve their health goals
Qualifications
  • Valid RN license in the state of CA
  • Minimum of 3 years of experience as a registered nurse is a plus
  • Experience in home health or a related setting is preferred
  • Hospice experience is a plus
  • Excellent clinical assessment and critical thinking skills
  • Strong communication and interpersonal skills
  • Ability to work independently and manage your time effectively
  • Compassionate and empathetic approach to patient care

  • Current BLS certification
