JOB DESCRIPTION:


The Skilled Nurse (LPN) role requires a compassionate, detail-oriented, and clinically capable Licensed Practical Nurse to provide high-quality, in-home skilled nursing services under the supervision of a Nurse Case Manager (RN).


This position plays a critical role in delivering hands-on clinical care to patients in the home, ensuring services are provided safely, effectively, and in accordance with physician orders and the established plan of care. The Skilled Nurse supports continuity of care, promotes patient stability, and contributes to a positive patient experience through consistent, reliable, and professional service.


RESPONSIBILITIES:


Skilled Nursing Care Delivery:


  • Provide in-home skilled nursing services as directed by the Nurse Case Manager and physician orders, including but not limited to:
  • Wound care and dressing changes
  • Medication administration and management
  • Vital signs monitoring and health assessments
  • Chronic disease support and patient stabilization
  • Basic treatments and procedures within LPN scope of practice
  • Deliver care in alignment with the individualized plan of care and agency standards
  • Observe, assess, and report patient condition and any changes to the Nurse Case Manager promptly


Care Plan Execution & Support:


  • Follow and implement care plans developed by the Nurse Case Manager
  • Reinforce patient education as directed (disease processes, medications, safety, etc.)
  • Support continuity of care by ensuring visits are completed timely and consistently
  • Assist in maintaining a safe home environment for patients


Communication & Team Collaboration:

  • Maintain clear, timely communication with the Nurse Case Manager regarding patient status, concerns, and updates
  • Collaborate with Home Health Aides, therapists, and other care team members to ensure coordinated care delivery
  • Interact professionally with patients, families, and referral sources


Documentation & Compliance:


  • Complete accurate, timely, and compliant documentation for each visit per agency standards
  • Follow all Medicare/Medicaid guidelines, payer requirements, and state regulations
  • Adhere to HIPAA, infection control, and safety protocols at all times
  • Ensure all care provided is within LPN scope of practice and under appropriate supervision

  • Deliver compassionate, patient-centered care that promotes dignity and independence
  • Support agency goals related to quality, outcomes, and patient satisfaction
  • Identify and communicate opportunities to improve care or patient experience


Travel Requirements:
