PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Fort Smith, Arkansas.

Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide high-quality patient care in the Ft. Smith, Arkansas area and surrounding communities. This role focuses on delivering skilled nursing services in patients' homes, conducting comprehensive assessments, completing OASIS documentation, and supporting patients throughout their care journey. The ideal candidate will have prior home health experience and be comfortable working independently in the field.

Schedule & Work Details

  • Location: Ft. Smith, AR and surrounding coverage areas
  • Pay Rate: $51.00 - $53.00 per hour
  • On-Call Requirement: One weekday every other week
  • Weekend Requirement: One weekend per month
  • Mileage Reimbursement: Mileage reimbursed between patient visits (from first patient to last patient)
  • Employment Type: Home Health Field-Based Position

Key Responsibilities

  • Provide skilled nursing care to patients in their homes according to physician orders and care plans.
  • Conduct comprehensive patient assessments and ongoing evaluations.
  • Complete and accurately document OASIS assessments.
  • Develop, implement, and update individualized care plans.
  • Monitor patient progress and communicate changes in condition to the interdisciplinary team and physicians.
  • Educate patients and caregivers on disease management, medications, treatments, and safety measures.
  • Maintain timely and accurate clinical documentation in accordance with regulatory and agency requirements.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
  • Participate in on-call and weekend coverage as required.

Required Qualifications

  • Active Registered Nurse (RN) license in the applicable state.
  • Recent home health nursing experience required.
  • Experience completing OASIS assessments.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and effectively manage a field-based caseload.
  • Valid driver's license, reliable transportation, and ability to travel throughout the assigned coverage area.
  • Current CPR/BLS certification.

Preferred Qualifications

  • Knowledge of Medicare home health regulations and documentation requirements.
  • Strong communication and patient education skills.
  • Experience working with diverse patient populations in a home care setting.
