About Us:
Home Instead in Fort Collins is a trusted provider of compassionate, high-quality home care services for seniors. We are seeking a dedicated Home Health Registered Nurse (RN) to support our Class A home care operations by providing nursing oversight, clinical assessments, and education to caregivers, clients, and families.
Position Summary:
The Home Health RN plays a vital role in ensuring our clients receive exceptional care in their homes. This position involves performing client assessments, developing care plans, providing caregiver training, and ensuring compliance with Colorado Class A home care regulations. The RN will collaborate closely with caregivers, families, and our office team to enhance the quality of life for our clients.
Key Responsibilities:
  • Conduct initial and ongoing client assessments to determine home care needs.
  • Develop and update individualized care plans in accordance with state regulations and client needs.
  • Provide medication setup, administration oversight, and training for caregivers and families.
  • Perform wound care, oxygen therapy, injections, and other skilled nursing tasks within home care scope.
  • Train and educate caregivers (CNAs and non-medical staff) on proper techniques, infection control, and emergency procedures.
  • Monitor client progress and collaborate with physicians, families, and interdisciplinary teams.
  • Ensure compliance with state regulations, company policies, and best practices for home health care.
  • Document assessments, care plans, and services provided in an electronic health record system.
  • Be available for on-call nursing support as needed.

Qualifications:
  • Active Colorado RN license (in good standing).
  • At least 1 year of experience in home health, geriatric care, or a related field preferred.
  • Knowledge of Colorado Class A home care regulations is a plus.
  • Strong clinical assessment and documentation skills.
  • Ability to train and support caregivers in a home setting.
  • Compassionate, patient-centered approach to senior care.
  • Reliable transportation and ability to travel to client homes in the Fort Collins area.
