What You'll Be Doing

  • Provide skilled home health nursing visits throughout the St. Paul service area
  • Perform comprehensive patient assessments
  • Deliver wound care, medication management, injections, and other skilled nursing services
  • Educate patients and caregivers on diagnoses, medications, and treatment plans
  • Monitor patient progress and communicate changes with physicians
  • Collaborate with therapists, social workers, and interdisciplinary care teams
  • Complete timely documentation in Homecare Homebase (HCHB)
  • Promote positive patient outcomes while helping reduce unnecessary hospitalizations

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
