We provide home health care under Medicare and Private Insurance to patients in their home; be it a single-family home, apartment building or assisted living. Our Registered Nurses, Therapists, and Social Worker coordinate care in Lake & McHenry County in Illinois and Kenosha, Racine & Walworth County in Wisconsin. We work with hospitals, skilled nursing providers and primary physicians to continuously assess medical conditions; ensure medication compliance by educating patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications; all while maintaining a level of care, compassion and consistency that dramatically improves the quality of life of our patients. We are very proud of the level of care and professionalism we provide and are seeking new energetic and motivated team members to join us as we continue to provide great home health care.
Job Description
Position requirements
  • Performs assessments and develops individualized patient care plans and manages multi-disciplinary care as applicable.
  • Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction with the plan of care.
  • Supervises LPN's and Home Health Aide's in a patient's home.
  • Complies with agency policies and procedures.
  • Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations.
  • Submits clear and appropriate documentation in a timely manner and documents patient education.
  • Communicates effectively to physicians, supervisors, and coworkers in a timely manner.
  • Participates in case conferences to promote coordination of care.
  • Promotes continuity of care with appropriate admissions, transfers and discharges.
  • The RN will attend in-service training and attend mandatory agency meetings.
  • Reports to appointments as scheduled.
  • Maintains a professional appearance
  • Perform all aspects of patient care.

Qualifications
  • Registered Nurse with current license in the state of Illinois, required.
  • Registered Nurse with current license in the state of Wisconsin, preferred.
  • One year of experience as an RN in an acute care setting, preferably two years of experience.
  • Must have at least one year of Home Health Experience.
