Pride Health is actively hiring a Registered Nurse (RN) to join our client's In- Home Hospice Team.

Pay rate: Up to $55.00/ hour (W-2)

Work Location: Bryant, AR 72022
Coverage Area: Pulaski, Saline, Jefferson & Surrounding Counties
Patient Care Setting: Hospice โ€“ Home-Based
Weekly Hours: 32 hours guaranteed
Productivity: 24 points/week
Schedule: 8:00 AMโ€“4:30 PM, Monday to Friday
On-Call/Weekend: Negotiable
Mileage: Reimbursed between patient visits. ($0.48)

Job Summary:

  • We are seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, high-quality care to hospice patients in their homes.
  • The RN will manage an assigned caseload, coordinate patient care, participate in interdisciplinary team (IDG) meetings, and travel daily between patients' homes.

Key Responsibilities:

  • Provide skilled nursing care to hospice patients in their home setting.
  • Perform patient assessments and monitor changes in condition.
  • Manage and coordinate care for an assigned hospice caseload.
  • Develop and implement individualized plans of care.
  • Participate in Interdisciplinary Group (IDG) meetings and collaborate with the care team.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals.
  • Provide education and support to patients and caregivers regarding symptom management and end-of-life care.
  • Complete accurate and timely clinical documentation.
  • Travel daily between patients' residences throughout the assigned coverage area.
  • Perform hospice needs assessments and admissions when required.

Required Qualifications

  • Registered Nurse (RN) with a minimum of an Associate Degree in Nursing (ADN).
  • Hospice nursing experience preferred.
  • HCHB (Homecare Homebase) experience required.
  • Excellent communication skills and bedside manner.
  • Ability to work independently and effectively manage a patient caseload.

Preferred Qualifications

  • Experience performing hospice needs assessments and admissions.
  • Strong case management and care coordination skills.
  • Experience providing end-of-life care and supporting patients and families.

Dress Code: Non-specific
Certification: RN License; minimum ADN required.

RN can work as a traveller as well. Traveller pay package can be discussed over a call.
