What You'll Do

  • Provide skilled nursing care, assessments, and treatments in patients' homes
  • Develop and manage individualized care plans for home health and/or hospice patients
  • Monitor patient progress and adjust care as needed
  • Collaborate with physicians, caregivers, and the interdisciplinary team
