You'll engage in various initiatives, driving quality assurance and improvement within the healthcare framework.**This position offers...*** **Employee Assistance Program** – 24/7 counseling services, legal assistance, & financial consultation for you and your household at no cost* **Mileage Reimbursement** – Company paid for work functions requiring travel* **Employee Discounts** – Hotels, Theme Parks & Attractions, College Tuition* **Workplace Culture** - An environment cultivating employee wellbeing, valuing each individual's humanity, and actively promoting a healthy, joyful workforce* **Additional Perks & Benefits** - Scroll down to bottom of this post to learn more**Key Responsibilities:*** Oversee and supervise the daily operation of the HCH Team.* Responsible for coordination and tracking of HCH enrollees (new enrollees, discharges, transfers).* Supervise Nurse Care Managers and assign NCM caseloads in partnership with the Assistant Director.* Coordinate and/or facilitate health education groups and/or activities for clients, staff, and community.* Review, complete, and submit the monthly Attestation report in CyberAccess in a timely manner.* Serve as a Cyber Access Practice Administrator maintaining user accounts and monitoring agency usage and prescriber reports.* Coordinate management of HIT tools and initiatives, including CyberAccess usage, CMT reports, and hospital admission/discharges.* Oversee and/or facilitate staff training on Healthcare Home.* Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings.* Oversee Quality Assurance and Quality Improvement for Healthcare Home.* Partner effectively with collaborating service areas to meet client needs.**Education, Experience, and/or Credential Qualifications:*** Current Registered Nurse (RN) in the state of Missouri is required.* Supervisory experience.**Additional Qualifications:*** Successful completion of the Criminal Record and Child Abuse & Neglect Background check prior to beginning employment.* Completion of New Hire Orientation within 30 Days of employment.* All training requirements including Relias Learning within two (2) weeks of employment and annually thereafter.* Strong verbal and written communication skills, computer, and organizational skills.**Physical Requirements:*** The physical requirements and working conditions described here are representative of those an employee encounters while performing the essential functions of this job.
