Job description

 

Part-Time Home Health RN working with individuals with developmental disabilities.

20 to 30 hours per week.

DUTIES:

  • Provide weekly assessments of individuals served
  • Conduct supervisory visits and document results
  • Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
  • Review of data tracking and documentation
  • Staff training and oversight including proxy care
  • Coordinate medical visits for clients
  • Medication reconciliations
  • Other duties as assigned
