The Role

The home health nurses will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

What You Will Do

  • Conduct comprehensive patient assessments in the home setting to evaluate the patient's physical and emotional health.
  • Develop and implement individualized care plans based on physician orders, patient goals, and clinical best practices.
  • Administer medications, injections, wound care, and other treatments as prescribed by the physician
  • Provide patient and caregiver education on disease management, medication usage, home safety, and recovery techniques.
  • Coordinate care with other healthcare providers, including physicians and therapists, to ensure continuity of care.
