Job description

RN - Home Health - Pinellas and Pasco County, FL

Seeking an experienced RN Case Manager to provide skilled home health visits and oversee patient care from admission through discharge.
Responsibilities include:
  • Completing OASIS assessments (SOC, ROC, Recertification, Transfer, and Discharge).
  • Developing and managing the plan of care, coordinating with physicians and interdisciplinary team members (PT, OT, ST, MSW, HHA, LPN).
  • Obtaining physician orders.
  • Reconciling medications, monitoring labs and DME.
  • Ensuring timely documentation, and providing patient/caregiver education. 
  • The RN is responsible for supervising LPNs and HHAs through required supervisory visits, reviewing clinical documentation, ensuring compliance with Medicare and agency standards, coordinating recertifications and discharges, and maintaining accurate, timely documentation in the EMR. 
Additional responsibilities include mentoring new field staff (LPN/RN), assisting with field and EMR training. Strong assessment, communication, organization, and home health/OASIS experience preferred.
