Job description

The Reentry Case Manager plays a key role in supporting successful community reintegration for individuals returning to Chatham, Johnston, Lee, Orange, and Wake Counties from federal correctional facilities. Serving as the primary point of contact for participants, this position ensures the comprehensive delivery of case management services for the Capital Area Workforce Development region under the NC PROWD initiative.


Key Responsibilities

  • Conduct individualized assessments and determine program eligibility.
  • Complete participant intakes and develop tailored service plans.
  • Coordinate supportive services-including basic needs assistance, transportation, employment support, and housing resources-to help participants overcome barriers to reentry.
  •  Collaborate closely with community organizations, workforce partners, and federal correctional facilities to maintain a consistent and streamlined referral and continuityofcare process.
  • Maintain accurate case notes, documentation, and performance data in compliance with program and grant requirements.
  • Advocate for participants to secure needed services and support.
  • Work within a multidisciplinary team to support decisionmaking, troubleshoot barriers, and implement service strategies.
  • Assist clients with navigating complex systems, resources, and community supports.

Please Note **This position is grantfunded through October 2028**. Continued employment is contingent upon the availability of future grant funding. If grant funding ends, the position will be discontinued.
