Are you a passionate Physical Therapist who wants to make a real impact-from pediatrics to geriatrics? Join our collaborative Rehabilitation Services team and help patients regain independence, mobility, and confidence through exceptional, evidence-based care.
What You'll Do
As a Home Health Physical Therapist, you'll deliver high-quality physical therapy services while working alongside physicians, nurses, and fellow rehab professionals to create meaningful outcomes for every patient.
Your day-to-day will include:
  • Evaluating patients and developing individualized treatment plans
  • Delivering hands-on therapy, therapeutic exercise, and functional training
  • Educating and motivating patients to reach their mobility and recovery goals
  • Utilizing a wide range of treatment modalities (manual therapy, hydrotherapy, ultrasound, traction, and more)
  • Monitoring progress and adapting treatment plans for optimal results
  • Collaborating on discharge planning and designing home exercise programs
  • Supervising Rehab Aides and supporting staff development
  • Maintaining accurate documentation and ensuring patient confidentiality
  • Promoting a safe, clean, and compliant treatment environment

What We're Looking For
  • Active Colorado Physical Therapy License (required)
  • BLS certification (required)
  • Previous experience preferred-but motivated new grads are welcome to apply
