Home Health and Hospice

Nursing
Req #: 72688

Are you looking for a nursing role that offers autonomy, meaningful patient relationships, and the opportunity to make a difference one patient at a time? Join the Care at Home team serving Lincoln County and become part of a compassionate group of professionals dedicated to helping patients receive exceptional care in the comfort of their own homes. In this 32-hour salaried position, you'll provide skilled nursing care, assess patient needs, coordinate services, and collaborate with physicians, therapists, and other members of the care team to help patients achieve their health goals. You'll have the opportunity to build lasting relationships with patients and families while delivering personalized, patient-centered care where it matters most.

Unlike a hospital setting, you'll manage your day independently, start and end your workday from home, and focus on caring for one patient at a time. This role offers the perfect balance of autonomy and teamwork, allowing you to make a meaningful impact while maintaining strong connections with colleagues and community resources. This position includes a weekend rotation of one weekend every fourth weekend, providing predictability and work-life balance while ensuring patients continue to receive the care they need.

This is a Registered Nurse 2 vacancy open to candidates with greater than 1 year of RN work experience.

For a limited time, MaineHealth is offering up to $10,000 Sign-on bonus for all eligible Registered Nurses with 0-2 years of experience and $20,000 for experienced Registered Nurses with greater than 2 years of experience!
o Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires, per diem hires are ineligible.
o Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment.

Summary:
The MH Registered Nurse - Home Care role provides patient-family centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care. Services may be provided on an outpatient and/or community basis.
MH RN II: This level is competent in their clinical role and has demonstrated the ability to care for most patients assigned. Though not able to care for every patient type within their area of hire, demonstrates how to obtain the appropriate resources to develop required skills and knowledge over time. This level included automatic advancement after 1 year as an RN I – must be in good standing and meets RN I requirements.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Graduate of an accredited School of Nursing required; BSN preferred.
  2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current BLS certification required or must obtain within 30 days of start date. Valid Driver’s License to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy required
  3. Experience: One year of RN experience required. Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where applicable)
