Job description


The Health at Home Registered Nurse is responsible for traveling to a patient's home to administer home health and hospice services and helping patients maintain their independence or allow them to remain in their home at end of life. The right candidate will possess strong interpersonal skills that allow them to get to know their patients, build a rapport, and establish trust to provide high quality care while improving the patient's quality of life and comfort. The home health/hospice nurse must be able to use sound clinical judgment to make decisions independently when required and accept direction and requests from patients. The home health/hospice nurse must be prepared to work in a variety of living environments and serve as an advocate for the patient.
The Health at Home nurse provides a variety of tasks during patent visits depending on the specific plan of care and type of patient, duties include but are not limited to:
  • Assess and document observations of patient condition
  • Complete evaluation, review medications, and obtain vital signs
  • Changing of dressings as ordered and assess wound/healing progress
  • Provide education to patients and families on proper home health strategies and procedures

Coordinate with the multi-disciplinary care team on progress related to the plan of care
