Required Qualifications
Mustresideinthestate of Illinois.
Registered Nurse in the state of Illinois without disciplinary action.
2+ years ofexperience ofin-home case/care management.
Experience working with the adult population.
Knowledge of community health and social service agencies andadditionalcommunity resources.
Exceptional communication and interpersonal skills with the ability to quickly build rapport.
Ability to work with minimal supervision within the role and scope.
Ability to use a variety of electronic informationapplications/softwareprograms including electronic medical records.
Intermediate toAdvanced computer skills and experience with Microsoft Word, Outlook, and Excel.
Excellent keyboard and web navigation skills.
Preferred Qualifications
Bachelor of Science in Nursing (BSN).
3+ years of in-home assessment and care coordination experience.
Experience with Medicare/Medicaid members.
Experience with health promotion, coaching, and wellness.
Previousmanaged care experience.

Workstyle/Travel:This is a Hybrid-Home position that requires occasional onsite work atthemarket office in Schaumburg,Illinois, aswell75-90% travel in the field to visit membersthroughout Illinois.
