Job Description Summary

We are seeking a Medical Social Worker to join our Home Health Department in Port Charlotte. In this role, you will deliver in-home social services to support patients' emotional, social, and environmental needs. Under the direction of the Director of Nursing, you will conduct assessments, provide counseling, coordinate resources, and contribute to individualized care plans while supporting our commitment to providing an exceptional experience for our patients, team members, and communities.

How will you make an impact & Requirements

What You'll Do

  • Conduct psychosocial assessments and evaluate home environments.
  • Provide counseling and support to clients and families coping with illness or life changes.
  • Assist clients in accessing community, financial, and family resources.
  • Participate in developing and implementing interdisciplinary care plans.
  • Document visits and clinical notes in the EMR within required timeframes.
  • Monitor and report changes in clients' emotional or social status.
  • Consult with physicians regarding care plan adjustments.
  • Coordinate weekly visit schedules with the Director of Nursing.
  • Participate in staff meetings, in-service education, and agency activities.
  • Provide monthly supervision to the Social Work Assistant.
  • Support discharge planning and continuity of care.
  • Maintain professional development and comply with agency policies.

What We're Looking For

  • Master's Degree in Social Work (MSW) from an accredited program.
  • Minimum two years of experience, including one year in a healthcare setting.
  • Strong interpersonal skills and a client-centered approach.
  • Experience with electronic medical records is preferred.
  • Requires strong communication skills and the ability to perform assessments in varied home environments.
  • Candidate must maintain current TB testing.
  • Occasional bending, lifting (up to 30 lbs.), and assisting with client mobility may be required.

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
