Work Environment

This position will operate primarily in a home care setting within the Charlottesville area, where you will engage with patients directly in their living environments. The nature of home care allows for personalized assessments and treatments tailored to individual needs. Key Responsibilities

• Examine, evaluate, and develop comprehensive treatment plans for patients dealing with various orthopedic conditions, focusing on improving mobility and functionality.
• Provide consultations to other healthcare providers regarding the continuum of care for patients, ensuring a cohesive team approach to treatment.
• Skillfully document all aspects of patient interactions, including evaluations, treatments, and outcomes, ensuring compliance with all applicable regulatory standards.
• Educate patients and their families about their conditions and treatment options, fostering a supportive and informative environment.
• Participate in community outreach initiatives to promote wellness and rehabilitation services, enhancing the visibility of the organization within the community.
• Continuously evaluate and re-evaluate patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

Patient Population / Client Focus

Your primary focus will be on patients with orthopedic impairments, including post-operative rehabilitation, frailty, and chronic pain management. You will work with a diverse demographic, catering to individuals of all ages, from young athletes recovering from injuries to elderly clients aiming to regain their mobility and independence. Your role is crucial in supporting these individuals on their journey to recovery, ensuring that treatment plans are culturally sensitive and respectful to each patient's unique background.

Qualifications

• A Doctorate in Physical Therapy is required, emphasizing a strong foundation in orthopedic rehabilitation theories and practices.
• Active licensure in the state of Virginia is a must, ensuring compliance with local regulations.
• Excellent verbal and written communication skills are essential for effective patient education and collaboration with other healthcare providers.
• Proficient in electronic health records (EHR) systems and capable of documenting treatment efficiently and accurately.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 100,000 – 120,000 per year, commensurate with experience and qualifications.
