American Traveler is hiring an experienced RN for a Hospice/Palliative Care position in a home health setting, requiring Homecare Homebase (HCHB) experience and prior hospice background.
Details- Home health / hospice care setting serving patients across Nelson, Spencer, and southern Bullitt counties in Kentucky
- Patient population includes both pediatric and adult hospice patients
- Monday–Friday, 8:00 AM–5:00 PM schedule with 8-hour shifts
- On-call required one weeknight per week and every 3rd weekend (includes both scheduled and on-call visits)
- Clinicians are responsible for calling patients directly to schedule visit times
- Daily responsibilities include Start of Care assessments, recertifications, and daily visits
- Charting performed in Homecare Homebase (HCHB) — experience is required
- No floating required
- Active KY RN license or Compact (NLC) RN license — must be active at time of consideration; pending licenses will not be accepted
- Current BLS certification required — pending certifications not accepted
- Minimum 1 year of RN experience required
- Prior hospice experience required — no exceptions
- Homecare Homebase (HCHB) EMR experience required — no exceptions
- Home health background required
- Valid driver's license and personal auto insurance required
- 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked — must include dates of employment and eligibility for rehire
- A copy of your driver's license is required for consideration
- Orientation is approximately 12 hours
- Home health/hospice aides are available to support the care team
- No holiday coverage required
