Job Summary:

We are seeking a compassionate, dedicated, and professional Registered Nurse (RN) to join our Home Health and Hospice team. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while promoting patient-centered care and interdisciplinary collaboration.

Key Responsibilities:


  • Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.
  • Develop individualized care plans in collaboration with physicians, patients, and families.
  • Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered.
  • Monitor, record, and report patient progress and changes in condition to physicians and other care team members.
  • Provide education to patients and families regarding disease management, medications, and home care techniques.
  • Coordinate patient care with interdisciplinary teams, including therapists, social workers, and home health aides.
  • Ensure accurate and timely documentation in the Electronic Medical Record (EMR).
  • Participate in on-call rotation as assigned.
  • Maintain compliance with all state, federal, and agency policies, procedures, and regulations, including HIPAA.
  • Provide compassionate end-of-life care and emotional support to patients and families in hospice settings.

Qualifications:


Education & Licensure:


  • Current and valid Registered Nurse (RN) license in the State of Texas (or compact state).
  • Graduate of an accredited School of Nursing (BSN or ADN).
  • Current CPR (BLS) certification required.
  • Valid Drivers License, auto insurance, and reliable transportation required.

Experience:


  • Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred).
  • Knowledge of OASIS and EMR charting preferred for home health nurses.
  • Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols.
