POSITION SUMMARY

The Registered Nurse (RN) provides professional nursing services in support of Ohio-licensed providers and home health agencies, including Ohio Adult Day Care DBA Peace Love Healthcare. The RN is responsible for the assessment, planning, implementation, and evaluation of nursing care for adult day care participants and home health clients in accordance with the Ohio Nurse Practice Act (ORC Chapter 4723), agency policy, physician orders, and the individualized plan of care. The RN coordinates care with the interdisciplinary team, supervises LPNs and home health aides, and serves as a clinical resource for direct-care staff.

ESSENTIAL DUTIES & RESPONSIBILITIES

Clinical Assessment & Care Planning

  • Conduct comprehensive nursing assessments, including OASIS assessments for home health clients, on admission, recertification, resumption of care, transfer, and discharge.
  • Develop, implement, and revise the individualized plan of care in collaboration with the physician, client, family, and interdisciplinary team.
  • Evaluate client response to care and modify the plan of care as needed; communicate changes in condition to the physician and other team members.
  • Document all assessments, interventions, and outcomes accurately and timely in the electronic health record.

Direct Nursing Care

  • Administer medications, treatments, and therapies as ordered by the physician, including IV therapy, wound care, injections, catheter care, ostomy care, tracheostomy care, and enteral feedings.
  • Perform skilled nursing procedures within the RN scope of practice as defined by the Ohio Board of Nursing.
  • Monitor vital signs, lab results, and clinical indicators; respond appropriately to abnormal findings.
  • Provide patient and family education on disease processes, medications, self-care, and safety.

Supervision & Coordination

  • Supervise the care provided by LPNs, Home Health Aides, and Personal Care Assistants in accordance with state and federal regulations, including 42 CFR 484.80 for home health aide supervision.
  • Conduct supervisory visits at required intervals; provide ongoing training, coaching, and competency evaluation.
  • Coordinate care with physicians, therapists, social workers, pharmacies, durable medical equipment providers, and community resources.
  • Participate in case conferences, quality assurance activities, and performance improvement initiatives.

Regulatory Compliance

  • Practice in accordance with the Ohio Nurse Practice Act, Ohio Administrative Code, Conditions of Participation for Home Health Agencies (42 CFR Part 484), and all applicable state and federal regulations.
  • Maintain confidentiality of all client information in compliance with HIPAA.
  • Report suspected abuse, neglect, or exploitation to appropriate authorities as required by Ohio law.
  • Participate in mandatory training, in-services, and continuing education.

MINIMUM QUALIFICATIONS

Required

  • Current, active, and unencumbered license to practice as a Registered Nurse in the State of Ohio (or compact state license valid in Ohio).
  • Graduate of an accredited school of nursing (Diploma, ADN, or BSN).
  • Current BLS / CPR certification (American Heart Association or American Red Cross).
  • Minimum of one (1) year of clinical nursing experience; experience in home health, hospice, geriatrics, or community-based care preferred.
  • Valid Ohio driver's license, reliable transportation, and current auto insurance.
  • Ability to read, write, and communicate effectively in English.
  • Proficiency with electronic health records (EHR) and basic computer skills.

Preferred

  • Bachelor of Science in Nursing (BSN).
  • OASIS certification (COS-C) or experience completing OASIS assessments.
  • Experience supervising LPNs and/or home health aides.
  • Experience with home health EHR systems (HCHB, Kinnser/WellSky, MatrixCare, etc.).
  • Additional certifications such as ACLS, wound care (WCC), or case management (CCM).

Physical Demands

  • Frequently required to stand, walk, bend, stoop, kneel, crouch, reach, and use hands and arms.
  • Must be able to lift, push, pull, and transfer up to 50 pounds independently, and up to 100 pounds with assistance or mechanical aid.
  • Must be able to safely assist with patient/client transfers, including the use of gait belts and mechanical lifts.
  • Manual dexterity sufficient to operate medical equipment, perform clinical tasks, and complete documentation.
  • Visual and auditory acuity sufficient to assess patient/client condition, read medical orders, and communicate effectively.
  • Must be able to remain alert and respond appropriately during the entire shift.

Work Environment

  • Work is performed in the Adult Day Care center, in client homes, or in other community settings throughout Ohio.
  • May involve exposure to bodily fluids, communicable diseases, household allergens, pets, tobacco smoke, and other environmental factors typical of community-based and home health care.
  • Must follow Standard Precautions and infection-control protocols at all times.
  • May involve travel between client locations using personal vehicle; valid Ohio driver's license, reliable transportation, and current auto insurance required when traveling.

PRE-EMPLOYMENT & ONGOING REQUIREMENTS

  • Successful completion of a criminal background check through the Ohio Bureau of Criminal Investigation (BCI) and the FBI, in accordance with Ohio Revised Code 109.572, 5164.34, and 5123.081.
  • Clearance through the Ohio Nurse Aide Registry, OIG List of Excluded Individuals/Entities, SAM.gov, and the National Sex Offender Public Website.
  • Verification of all required licensure, certification, and educational credentials.
  • Negative pre-employment drug screen and completion of a post-offer physical and two-step TB test (or current documentation).
  • Current Hepatitis B vaccination or signed declination.
  • Compliance with all federal, state, and agency requirements for ongoing training, in-service hours, and credential renewal.
