Job description

THE ORGANIZATION 

Outer Cape Health Services (OCHS) provides a full range of primary health careandsupportive social servicesthat promote thehealth and well-beingofall who live in or visit the ten outermost towns of Cape Cod. With locations in Harwich Port, Wellfleet and Provincetown, OCHS cares for more than 20,000 patients annually, and no one is denied access to services due to an inability to pay.  

Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966). As a Patient-Centered Medical Home, OCHS holds itself to the highest standards.Ensuring patients have access to care when they need it and request it and validating that staff are working at the top of their licensures are foundations of this model. Primary Purpose of Position: 

Reporting to the Director of Behavioral Health, the Behavioral Health Clinician is contracted with the Town of Eastham and serves as a municipal mental health professional for residents of all the four Outer Cape towns of Eastham, Wellfleet, Truro and Provincetown. This role functions as a member of OCHS and collaborates with a multi-disciplinary team at the municipal level as well as with clinicians for all OCHS patients. The Behavioral Health Clinician may evaluate need for hospitalization, make referrals, and provide immediate follow-up services to support an individual’s engagement in treatment if needed, as well responsibility for providing resources and treatment to patients, collaborating with other members of staff to create care plans, conducting screenings, and case management. 

Essential Functions: 

  • Provide direct services to youth and families, including information and referral, assessments, coordination of treatment, and ongoing therapeutic interventions through individual, family and/or group modalities.   

  • Apply immediate community based clinical services when and where needed based on referrals and in partnership with public safety departments responding to behavioral health calls. Prioritizing individuals who are unable to receive safe, timely and adequate services through healthcare agencies or other health and human services providers that would provide similar case management 

  • Provide clinical expertise and assistance to the community concerning the process of accessing appropriate medical intervention, as well as interventions, rehabilitation and recovery issues, and the integration of clients back into the community. 

  • Provide counseling session services to individuals and families of the Outer Cape towns to those impacted by addiction, substance abuse and related mental health issues. Develop and assist in treatment planning and locating available services , provide case management and followup support. 

  • Provide outreach service and capacity to meet in person or conduct telehealth. 

  • Work and collaborate with assigned team to implement joint care plans, as applicable, and balance a mix of short term, maintenance, group, individual and extended therapeutic modalities to ensure ongoing access and best possible outcomes. 

  • Insure that OCHC patients are screened using appropriate screening tools (CANS, PHQ-2,9) and are properly scored and that patient care is directed accordingly.   

  • Collaborate daily with healthcare team and other OCHS staff to assure that OCHS clients with behavioral health issues are treated in a coordinated and integrated fashion, moving towards the overall goal of full integration between primary care and behavioral health at OCHS. 

  • Maintain case records in accordance with OCHS policies and procedures. 

  • Interacts with referral sources and other internal and external sources as necessary to ensure coordination and access to services for patients with behavioral health needs. 

  • Participate in staff supervision, in-service trainings and other OCHS meetings as directed by the Director of Behavioral Health. 

  • Other related duties and projects as assigned. 

Non-Essential Functions: 

  • Attend periodic staff meetings.  

  • Adhere to OCHS policies. 

  • Adhere to OCHS values and principles. 

Qualifications: 

  • Master’s degree in in Clinical Social Work or related fields. 

  • Three to five years’ experience in behavioral health services preferred. 

  • Two years’ experience providing behavioral health services in Community Health Center preferred. 

  • Bi-lingual and bi-cultural capabilities preferred. 

  • Experience in Substance Use Disorder Treatment preferred. 

  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting. 

  • Excellent written and verbal communication skills – bi-lingual helpful 

  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive and any other software applicable to this position. 

  • Excellent judgment and ability to solve problems in a timely manner. 

  • Ability to adhere to strict confidentiality standards. 

  • Demonstrated ability to work effectively both as a team member and independently. 

  • Demonstrated ability to work in a culturally diverse and inclusive environment. 

Physical Requirements: 

This is a sedentary job by nature. Requires being able to sit and/or stand for long periods of time as well as the ability to use computers and a phone.  

Internal and External Contacts: 

Interfaces regularly with employees and patients in person, on the phone, and via email.
