Active, Current, Unrestricted state Medical Doctor or Doctor of Osteopathy license from any of the 50 United States and Commonwealth of Puerto Rico. 4. Active, Current, state license for Physician Assistant or Nurse Practitioner 5. Active, Current, DEA Registration 6. The candidate is not under any sanction or exclusion from Medicare I Medicaid programs. 7. The candidate has provided Emergency Department medical care for the period of 12 consecutive months prior to presentation 8. Emergency Medicine Board Certified or eligible 9.
