Hopi Health Care Center & Fort Yuma HCC

Polacca, AZ & Winterhaven, CA โ€” Indian Health Service

The Hopi Health Care Center is located in Polacca, Arizona, on the Hopi reservation in northeastern Arizona. The area offers stunning high-desert landscapes, rich Native American cultural heritage, and proximity to attractions like Petrified Forest National Park and the Painted Desert. Flagstaff, with full amenities, dining, and outdoor recreation, is approximately 90 minutes away.

Hopi Health Care Center (HHCC) is an AAAHC-accredited Indian Health Service facility serving approximately 18,000 patients. Fort Yuma Health Care Center (FYHCC) is an Client facility in Winterhaven, CA serving the Quechan Tribe community.

Position Overview

We are seeking a Family Nurse Practitioner to provide same-day and acute ambulatory primary care services. This is a nonpersonal services contract position.

Responsibilities
  • Evaluate, diagnose, treat, and disposition patients presenting for same-day or acute ambulatory care
  • Order and interpret diagnostic tests; prescribe medications and therapies
  • Perform minor procedures within scope of practice and granted privileges
  • Document encounters in Government EHR system
  • Coordinate referrals and consultations as needed
Requirements
  • Master's degree from accredited NP program
  • Current, active, unrestricted NP/APRN license (any US state)
  • National FNP certification (AANP or ANCC)
  • BLS required; ACLS preferred
  • DEA registration with prescriptive authority
  • Minimum 2 years primary care experience preferred
  • Client/tribal/rural healthcare experience preferred

Monday-Friday, business hours per BPA Call assignment
