Data Collection: Unit Information: (insert name) Location: Address: 2425 Samaritan Drive, San Jose CA 95124 Floor: Basement Building: Main Hospital General Information Tell us about the unit(s): • Number of Beds - 4 Cath Lab Rooms • Number of Staff - 20 RNs • Type of staff: Room Mix 2 RN and 2 RT. 8 - 9 RNs staffed daily o Day Shift - 7 - 3:30 PM Monday - Friday o Night Shift - On call only • Patient Ratios - 2RN + 2RT per patient in procedure • Type of equipment - GE + Philips • EMR - MacLab - Meditech Typical hiring profile Skill Set Most recruited for: Must have: candidates without these skills will not be considered for the role. • ACLS, BLS, NIH, Current RN license • Preferred or nice to have: candidates with these skills will be considered first. • Neuro and Electrophysiology experience • List typical procedures performed on unit(s): • Cardiac Cath lab, heart Cath's, Peripheral procedures, A-fib/Flutter - SVT, VT etc. • Stroke and complex Neuro intervetions Best personality Fit: • Team work REQUIRED CERTIFICATIONS: • ACLS, BLS, Current RN license Floating Area(s): • Recover cath lab patients Dress Code: • Scrub Color/Attire - Scrubs provided by hospital Unit Guidelines/Policies: Please feel free to attach unit policies and guidelines. • Weekends and On call(if any) • On call by rotation if need arises.
