• Conduct thorough assessments of mental health needs for patients in hospice care, tailoring treatment plans to support emotional well-being and coping mechanisms during end-of-life transitions.
• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and supportive counseling to provide personalized interventions for individuals, couples, and families.
• Facilitate support and psychoeducational groups that address grief, loss, and mental health concerns common among patients and families in hospice settings.
• Coordinate care with a multidisciplinary team, including social workers, nurses, and medical professionals, to ensure a holistic approach to patient treatment.
• Provide crisis intervention services when necessary, offering immediate support to patients facing acute emotional distress.
• Educate patients and their families about mental health conditions, coping strategies, and available community resources that can assist throughout the hospice journey.
• Maintain meticulous documentation regarding patient care in compliance with HIPAA and other regulatory standards, ensuring confidentiality and legal adherence.
• Engage in continuous professional development by staying abreast of the latest research and best practices in mental health therapy relevant to hospice care.

The core of your work will involve supporting patients in hospice who often experience complex emotional challenges such as grief, anxiety, and fear about dying and loss. You'll work closely with patients facing terminal illness and their families, providing emotional support that is essential for navigating this difficult period with dignity and understanding.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• State licensure as an LCSW, LPC, or LMFT, demonstrating your professional qualifications to provide mental health services.
• A deep understanding of mental health disorders and the therapeutic techniques best suited for patients in hospice.
• Proven expertise in conducting mental health assessments and creating effective treatment plans for diverse populations.
• Exceptional interpersonal skills with a strong emphasis on empathy, active listening, and ethical practice standards.
• Proficient in using electronic health record (EHR) systems for patient documentation and care coordination.

• At least two years of direct experience in mental health therapy, particularly in environments focused on end-of-life care or palliative support.
• Experience with crisis management and intervention, equipped to handle acute mental health issues in highly sensitive situations.
