Works with Physicians in standardizing order sets and clinical pathways.
2. Responsible for the development, revision, and implementation for all stroke and spine patient education materials and notebooks to educate patients and families while incorporating an interdisciplinary approach and utilizing best practices and evidence-based research. Ensures all materials are standardized and consistent with standard order sets and clinical pathways. Accountable for working with physicians, nurse leasers and staff to update on an annual basis or as needed to maintain high quality of care standards.
3. Accountable for the key performance measures of the Stroke Center and Spine Program. Works with the team to identify and track key performance measures, including, but not limited to: patient satisfaction, physician satisfaction and patient outcomes. Collects and aggregates outcome data on discharge phone calls. post op pain, complications, infection, PE/VTE, LOS and readmission. Summarizes and reviews outcome data in monthly interdisciplinary team meetings with key stakeholders and participates in process improvement initiatives
4. Oversees all regulatory activity related to the Joint Commission certification as a Disease-specific Care program in collaboration with the BMC and/or JMC quality department of regulatory readiness and ensures compliance with standards.
5. Provide expert nursing care consultation and bedside support expertise in comprehensive stroke treatment in the emergency departments, intensive care unit, and neuro/ortho floor. Serves as a resource to nurse managers in the identification of staff learning/performance needs. Works with physicians to bridge educational needs and programs for implementation of new technology.
6. Conducts the preadmission education class for patients scheduled for spine surgery to ensure that all patients receive appropriate education regarding their surgical procedure, including preparation for hospitalization, what to expect during the hospitalization and following transition to home, skilled or rehabilitation services. Works with Case Management for discharge preparation to ensure a comprehensive patient education is achieved and maintained on an ongoing basis. Oversees the coordination of services for all pre-op spine surgery patients, including coordination of physician offices to ensure timeliness of diagnostic services and preoperative work ups are complete, follows all patients and coordinates patient care during the acute hospitalization, and post discharge follow up to facilitate a seamless delivery of care process
7. Functions as a member of an integrated professional team in the ongoing accurate assessment of patient's needs through rounding with physicians and nurse caregivers to promote the development, implementation, and evaluation of the nursing care plan on an ongoing basis. Collaborates with physicians, nursing, physical therapy, operating room staff, marketing department, food service, pharmacy, case management and all members of the healthcare team through team conferences and one-on-one meetings to ensure a quality care delivery process for patients across the continuum
8. Responsible for stroke and spine database, conducts data collection, data analysis and development of reports and provides statistical information to the team related to spine and stroke care and clinical indicators.
