Employment Status:Variable / FlexShift:Day (United States of America)Facility:100 Spotswood Dr - LexingtonRequisition Number:R156123 Home Health Nurse- RN- Flex (Open) How You'll Help Transform Healthcare:As part of the health care team, the Home Health Nurse assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.

Flex schedule. Orientation period is consistent M_F schedule for first 4-6 weeks, then schedule moves to two days a week.


As part of the health care team, the Home Health Nurse assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.

  • Assesses, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients of all ages as appropriate to service area.
  • Age specific admission and ongoing assessments are completed, documented and used to develop a plan of care.
  • Collaborates with members of the health care team/patient/family to develop, implement, and evaluate an age-specific interdisciplinary plan of care. Revises plan according to changes in patient status.
  • Treatments and medications for patients of all ages are accurately administered and documented utilizing automated computer systems, including electronic medical records.
  • Patient/family (caregiver) educational needs are identified in a timely manner, incorporated into the plan of care, implemented and documented appropriately.
  • Determines and performs within clinical, leadership, and/or education pathways to support (direct/indirect) evidenced based, quality patient care service and performance improvement.
  • Exhibits clinical expertise in rapidly changing circumstances.
  • Plans strategies to reduce length of stay, turn around time, resource consumption and improve patient outcomes.
  • Functions as role model for clinical practice to coworker, students, and physicians.
  • Manages resources, establishes priorities, and modifies environment to meet patient care needs.
  • Completes manager/director predetermined projects within specified time frame and according to preestablished guidelines.
  • Uses problem-solving methods to facilitate resolution to patient and personnel situations that present in the work area.
  • Takes an active role in the orientation of new employees.

Interacts with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care. Delegates and oversees delivery of patient care. Maintains patient, staff and customer confidentiality. Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient's status. Interprets the appropriate information needed to identify each patient's requirements relative to his or her age specific needs and provides care according to departmental policies and procedures. May be exposed to infectious diseases, bio-hazardous and hazardous materials, including regulated medical wastes, and combative patients. Work is performed in a patient care environment and is often fast-paced, requiring use of prioritization skills. Physical requirements include: lifting, prolonged periods of standing, bending, pushing, pulling, reaching, walking, etc. May be asked to provide care at other Carilion facilities to meet patient needs. Some positions may be required to take call. Supports the Culture of Excellence and actively practices the Carilion Clinic Mission, Values, and Guiding Principles. Some positions may be required to take call.

Career Advancement Program: RNs are eligible to participate in the Career Advancement Program (CAP) to attain various levels of advancement. Levels are determined by achievements as outlined in the CAP Scoring Sheet, which is part of the RN performance appraisal.

What We Require:

Education: Graduate of school of professional nursing.
Experience: One (1) year experience in hospice, home health, oncology, or medical surgical nursing required.
Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the Commonwealth of Virginia. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law.

Life Support: AHA BLS-HCP required.
Other Minimum Qualifications: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
