Job description

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient's care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: Monday - Friday 8:00am-5:00pm, with varied hours for coverage as needed
Incentive: $48/hr per diem for each day worked, in addition to base pay
Position Summary:
The Travel Flex Nurse is responsible for traveling to various locations throughout BCN service areas to provide nursing coverage as needed. This role requires cross-training and the ability to deliver patient care in multiple settings, including routine home care, inpatient units, and nursing facility environments.
The Travel Flex Nurse provides skilled, professional nursing care to patients, families, and caregivers as prescribed by the physician and in compliance with State Board of Registered Nursing regulations, patient needs, and the policies and procedures of Bluegrass Care Navigators.
Position Qualifications:
โ€ข Graduate of an accredited school of Registered Nursing.
โ€ข Current Registered Nurse licensure in Kentucky or a participating Nurse Licensure Compact (NLC) state.
โ€ข Minimum of one year of general nursing experience required.
โ€ข Demonstrated clinical competence with the ability to maintain professionalism during emotionally challenging situations.
โ€ข Compassionate, respectful caregiver with experience serving diverse populations.
โ€ข Knowledge of and commitment to hospice philosophy, including principles of death and dying.
โ€ข Current CPR certification required.
โ€ข Strong assessment, critical thinking, communication, and documentation skills.
โ€ข Ability to work independently with minimal supervision while functioning effectively within an interdisciplinary team.
โ€ข Flexible and cooperative to meet patient needs, including availability for a 24/7 operation and travel within defined BCN service regions.
โ€ข Willingness to provide care across multiple settings, including private homes, nursing facilities, and inpatient units.
โ€ข Ability to meet employment requirements, including satisfactory references and compliance with BCN employee health, infectious disease, and vaccination policies.
โ€ข Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
