  2. Photocopy of Criminal History Clearance letter, OR PAYMENT RECEIPT.
  3. School Nurses Only must ALSO PROVIDE - Photocopy of New Jersey RN License, CPR, and AED current registration, and ELECTRONIC (directly from College's Clearing House)/OFFICIAL undergraduate TRANSCRIPTS (in originally sealed envelope).
NOTE:

  • Photocopy of New Jersey Substitute Credential or NJ Teacher Certification.(FOR APPLICANTS IN POSSESSION OF A TEACHING OR SUBSTITUTE CERTIFICATE)
  • A completed substitute application available on Applitrack https://www.applitrack.com/tenafly/onlineapp/ .
  • I-9 Form - Acceptable documents are noted on the form and must be presented to Jeannine Wright (photocopies are not acceptable).
  • W-4 Forms, State & Federal
  • Physical Examination
  • Mantoux Test for tuberculosis - proof of negative result.
  • New Jersey First Act Form
  • Fingerprinting (https://homeroom4.doe.state.nj.us/chr/). (previously fingerprinted substitutes must TRANSFER their fingerprints)
  • Verification of Continuous Employment completed & signed by District where prints originated (for previously fingerprinted substitutes)
