In this role, you will provide direct patient care, focusing on promoting wellness, managing chronic conditions, and coordinating care with other healthcare providers.   • Patient Care and Assessment: Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents, focusing on preventive care and chronic disease management.

  • Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
  • Masters Degree in Nursing or Physician Assistant Studies
