Role Summary

The Certified Nursing Assistant (CNA) – Geriatric Care provides direct personal care and routine daily living support to elderly patients within a clinical or residential care environment.

Duties & ResponsibilitiesDirect Patient Care & Daily Living Support
  • Activities of Daily Living (ADLs): Assist patients with daily routines, including bathing, dressing, grooming, oral care, toileting, and personal hygiene.

  • Mobility & Transfers: Assist patients with ambulation, transfers (bed to chair/wheelchair), and body re-positioning using gait belts and mechanical lift equipment.

  • Nutrition & Hydration: Distribute meals, assist patients with feeding, monitor fluid intake, and document meal consumption.

  • Vital Signs & Monitoring: Measure and record vital signs, including blood pressure, pulse, respiration, temperature, oxygen saturation, and weight, reporting changes to the supervising nurse.

  • Patient Observation: Observe and report changes in patient physical condition, skin integrity, cognitive status, or behavior to the clinical team.

  • EHR Documentation: Log daily care activities, vital signs, intake/output, and behavioral observations into the Electronic Health Record (EHR) system.

  • Compliance & Safety: Maintain compliance with established care plans, infection control standards, safety protocols, and HIPAA regulations.

  • Other duties as assigned.

Required QualificationsLicensure & Certifications
  • Certification: Active Certified Nursing Assistant (CNA) certification in the state of practice.

  • Life Support: Active CPR / Basic Life Support (BLS) certification.

  • Work Experience: Minimum one (1) year of CNA experience working with a geriatric or memory care population.

  • Physical Demands: Ability to stand, walk, bend, stoop, and lift/transfer patients weighing up to 50 pounds using proper body mechanics.

Desired Qualifications
  • Work Setting: Prior CNA experience in an Adult Day Health Center, Skilled Nursing Facility (SNF), or long-term care setting.

  • Specialized Training: Experience working with individuals diagnosed with Alzheimer's disease or dementia.

  • Language Abilities: Bilingual verbal skills in English and Spanish or Cantonese.
