Diabetic Case Manager - AccessHealth Horry
POSITION SUMMARY:
The Diabetic Case Manager coordinates care and services for the low income, uninsured population of Horry County within the community, and is responsible for enrollment, assessment and management of AccessHealth Horry (AHH) clients with an emphasis on psychosocial wellness, medication assessment and reconciliation, planning care, and health coaching. This position will report directly to the AHH Executive Director.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Case manage AccessHealth Horry diabetic patients by linking them to available health care resources and providing education/follow up for the purpose of lowering their A1c level.
  • Work collaboratively with existing community partners/partnerships to assure timely screening of all patients for: State and Federal program eligibility; Social Determinants of Health; and to initiate achievable and individualized plans of care.
  • Assist the uninsured patients by scheduling doctor appointments, coordinating care, and going with the patient (if needed) to assure compliance. Visits to hospitals, community partners' offices and home visits will be performed.
  • Be responsible for program evaluation by utilizing established program metrics and outcome measures.
  • Other duties as assigned.

EDUCATION and/or EXPERIENCE:
  • Bachelor's degree from an accredited school in a human service-related field and one year of experience in a health care or social service setting.
  • Prior Case Management experience preferred.
  • Ability to speak both English and Spanish preferred.
  • General knowledge of human anatomy.
