The Skilled Nursing Facility supports Villagers with complex medical needs while promoting independence, quality of life, meaningful relationships, and personal choice. Through compassionate care and clinical excellence, our Nursing team creates an environment where Villagers are known as individuals and supported in living meaningful and fulfilling lives.


Position Purpose

The Registered Nurse (RN) – Charge Nurse provides clinical leadership, supervision, and hands-on nursing care within Annandale Village’s Skilled Nursing Facility, ensuring Villagers receive safe, compassionate, and person-centered care.

As a nursing leader, the RN – Charge Nurse directs day-to-day nursing activities, exercises independent clinical judgment, supports and supervises nursing employees, and coordinates care to ensure services meet Villager needs and applicable regulatory and professional standards. This is a working nursing leadership position. The RN – Charge Nurse remains actively involved in medication administration, treatments, assessments, documentation, and direct Villager care while providing visible leadership and clinical guidance to the nursing team.


Major Duties and Responsibilities


Providing Person-Centered Nursing Care

  • Provide safe, compassionate, and person-centered professional nursing care within the RN scope of practice.
  • Perform and supervise medication administration, prescribed treatments, therapies, assessments, and other clinical services.
  • Monitor Villager health status and use clinical judgment to recognize, respond to, document, and communicate changes in condition.
  • Evaluate Villager health concerns and coordinate with physicians and other healthcare providers to support timely treatment and resolution.
  • Provide direct hands-on nursing care as needed to support Villagers and the nursing team.
  • Advocate for Villager health, comfort, dignity, preferences, rights, and quality of life.


Leading the Nursing Team

  • Direct and supervise day-to-day nursing activities to ensure safe, timely, and high-quality Villager care.
  • Provide leadership, clinical guidance, instruction, and support to nursing employees throughout the assigned shift.
  • Exercise independent judgment in addressing clinical, staffing, employee, and operational concerns within the scope of the role.
  • Support the orientation, training, and development of new and existing nursing employees.
  • Monitor nursing documentation and care delivery for accuracy, timeliness, completeness, and compliance with established standards.
  • Promote accountability, teamwork, professionalism, and person-centered care throughout the Nursing team.


Coordinating Clinical Care and Medication Management

  • Receive and implement physician orders and ensure medication and treatment records are updated appropriately.
  • Coordinate medication ordering, delivery, verification, and other medication-management processes.
  • Communicate significant changes in Villager health status and other appropriate medical information to families and guardians.
  • Coordinate Villager admissions, transfers, and discharges and participate in the development and implementation of individualized care plans.
  • Collaborate with physicians, healthcare providers, Culinary Operations, Nutrition, and other members of the interdisciplinary team to support coordinated care.
  • Ensure Villager preferences, needs, and goals are considered as part of clinical decision-making and care planning.


Quality, Safety, and Compliance

  • Ensure nursing services are provided in accordance with applicable federal, state, and local requirements, professional standards, organizational policies, and established procedures.
  • Maintain Villager privacy and confidentiality and ensure nursing employees understand and uphold Villager rights.
  • Participate in the development, implementation, and ongoing improvement of nursing policies, procedures, and clinical practices.
  • Identify and respond to concerns that may affect Villager health, safety, quality of care, or regulatory compliance.
  • Participate in quality assurance, safety, professional staff, and other committees or improvement initiatives as assigned.
  • Maintain professional competencies through continuing education, training, and ongoing learning.


Communication and Teamwork

  • Maintain open, respectful, and timely communication with Villagers, families, guardians, healthcare providers, employees, and other members of the interdisciplinary team.
  • Promote effective communication among nursing employees and across shifts to support continuity and consistency of care.
  • Collaborate with nursing leadership and other departments to address Villager needs and support quality outcomes.
  • Participate in staff meetings, care planning, required training, and other organizational activities as assigned.
  • Foster a positive work environment centered on communication, teamwork, accountability, and compassionate service.


Other Responsibilities

  • Maintain required licensure, certifications, training, and competencies.
  • Support departmental planning and other Nursing initiatives as assigned.
  • Follow Annandale Village policies, procedures, safety standards, and applicable regulatory requirements.
  • Perform other duties in the best interest of Annandale Village and the Villagers we serve.


Required Qualifications

  • Graduate of an accredited school of Registered Nursing.
  • Current and unrestricted Registered Nurse (RN) license in the State of Georgia.
  • Three to five years of recent RN experience in Georgia, with supervisory experience required.
  • Ability to supervise and instruct others and exercise independent judgment.
  • Ability to read and write in English, understand and follow written and verbal instructions, and communicate effectively with Villagers, employees, families, healthcare providers, vendors, and consultants.


Employment Requirements

  • Must be at least 18 years of age.
  • Valid Georgia Class C Driver’s License, proof of vehicle insurance, and acceptable MVR.
  • Must meet Annandale Village requirements for operating organizational vehicles and transporting Villagers.
  • Must pass GBI and FBI background checks, including fingerprinting.
  • Must pass drug screening and tuberculosis screening.
  • Must maintain current RN licensure and all required training and certifications.


Physical Requirements

  • Ability to sit, stand, walk, bend, reach, climb, balance, stoop, kneel, crouch, and perform nursing responsibilities throughout the workday.
  • Ability to provide hands-on nursing care and respond appropriately to clinical and emergency situations.
  • Ability to assist Villagers with mobility, positioning, transfers, and other individualized care needs.
  • Ability to lift up to 25 pounds independently and move up to 75 pounds with assistance.
  • Ability to safely transport and assist Villagers into and out of vehicles as required.
  • Ability to perform CPR, First Aid, and emergency response duties.
