Thrive Homes, Inc. is seeking a Family Model Provider located in Dyer County.

Family Model Provider

Thrive Homes is approved to provide Family Model services to both the DIDD Waiver and ECF/Choices Enrollees. Family Model Providers are needed to provide room and board in their own homes (apartment or house) to one or two people with disabilities. The Family Model Provider will be responsible for providing person-centered direct personal care including activities of daily living such as cooking, bathing, cleaning, medication administration assistance, and medical appointments. Providing support that will help the people supported to reach their goals and outcomes as per the PCSP is paramount to the well-being, independence, and growth of the people supported.

Qualifications and Experience

  1. Must have at least 1 year experience working with people with intellectual disabilities.

  2. Must pass criminal background check

  3. Must have a decently owned or rented home or apartment with a dedicated bedroom for the people supported. Must be wheelchair accessible if the person supported uses a wheelchair.

  4. Must have a reliable vehicle with auto insurance and a driver's license

  5. Must complete all required training, including but not limited to Relias, Medication Certification, and CPR/FA.

  6. Must have/obtain a computer with internet access for documentation purposes.

  7. Highschool graduate of equivalent preferred.
