Job description

DIRECT SUPPORTS PROFESSIONAL SUPERVISOR


Qualifications:

Must be at least 21 years of age.

Must have at least one year experience working with individuals that have intellectual/developmental disabilities and/or acquired brain injuries.

Must have good management skills

Must have good communication skills

Must meet all expectations of any employee hired through BluWell of Kentucky.


Requirements: Must be able to multi-task.

Must be able to handle stressful situations

Must be able to transport clients

Must cover extra shifts in the event of a call in and no one else can cover.

Must work following hours: Must work 40 hours and/or as scheduled by the Residential Coordinator.

Manage and supervise all staff in residential settings.  Be able to accept responsibility for any actions occurring from the staff or self in the residential setting. Be willing to investigate problems and occurrences out of the ordinary that have been reported.  Provide regular on-site job coaching, support and supervision of all DSP staff.  Ensure DSP staff have the necessary skills and knowledge to provide exceptional care to individuals and are following agency policy.  Be available to DSP staff by telephone as needed for technical assistance while they are on shift.

Ensure a high level of customer satisfaction among BluWell customers, including individuals, family members, guardians, case managers, and other agencies.

Ensure each individual is being supported by DSP staff in a manner that is consistent with his/her individual needs, preferences, and desires.  Ensure each individual's plan of care is being carried out in the residential setting. 

Conduct random house inspections as designated by the Residential Coordinator. Provide required documentation to the Residential Coordinator from these checks. Be willing to correct, in a positive manner, the problems that have occurred or been noticed and accept responsibility for these actions. Hold staff meetings and in-service trainings to ensure that problems are addressed and corrected.

Monitor all medications in any staffed residence, even if a client self- medicates. Report to Nursing Department all medications needing to be refilled, or new to the home. As well as make sure all scripts are current and exactly match what the medication given is.  Continually check and monitor MAR's for correct printing and doses. Residential Team Leader must review medication counts and ensure that individuals will not run out of medication at any time. Be sure that there is a refill coming or call in a refill to ensure not running out.  Ensures a minimum of five (5) days of medications are present in the home at all times.

Make and transport to doctors' appointments according to when needed. Transporting to appointments may be designated to appointment staff, but the Residential Team Leader has the responsibility of overseeing this and ensuring all appointments are made and kept.  This includes medication refills, as well as physicals and any routine appointments.  After doctors' appointments are completed, ensure prescriptions are placed in client files and a copy give to the Nursing Department, and ensure the pharmacy has the scripts to fill and deliver.  Ensure medication is delivered to the home and given as directed by the physician.

Be aware of all medical needs of any individual residing in the home, as well as be sure their medical needs are taken care of properly and timely. This includes ensuring DSPs receive training on specific medical conditions and equipment related to the individual.

Attend all scheduled Team Meetings for individuals living in assigned homes. Residential Team Leader input is essential to team meetings.

Serve as primary contact for outside agencies and families for the individuals in assigned homes.  Return phone calls and emails in a timely, professional, and courteous manner.  Remember that you are representing BluWell of Kentucky and that your behaviors directly reflect on how others perceive BluWell of Kentucky

Be able to train all staff on policies of BluWell of Kentucky DSP roles in the house and the duties expected on shifts, and any documentation needing to be completed in the home.

Follow-up and makes sure DSP staff is completing all documentation correctly. If not completing documentation correctly, hold an in-service with all staff in the home and have all staff sign in. Make sure that all staff are aware of the correct way to fill out documentation before ending the meeting or in-service.

Hold monthly house meetings to ensure all staff in the home are able to express interests, problems occurring with other staff, individuals, or questions they might have. Turn in training documentation to the Human Resources and Training Coordinator on the date, length of training and topics covered in the meetings and ensure all DSP staff in attendance have signed the sign-in sheet.

Alert the Residential Coordinator and Executive Director of any major concerns or repeated problems occurring so that they can address the issue at a different level. This will occur after you have tried to address the situation yourself.

Turn in all documentation no later than the 3rd of the following month. This will include water temps, outcome sheets, attendance log, MARs, etc. Any incident reports need to be turned in the next business day. Ensure all incident reports are turned in on time and are completed correctly by the staff before submitting to the office.

Complete a monthly residential summary for each individual in assigned houses no later than the 15th of each month for the previous month.  Provide case managers and guardians with copies of residential summaries as requested.

Ensure each individual's office and home binder is complete, up to date, and contains all documentation required by Medicaid wavier regulations and agency policy.

Attend all trainings requested by BluWell of Kentucky. This could include DDID trainings, BluWell of Kentucky trainings, individual specific trainings or management skills training. As a part of trainings, you will also need to hold a training session in the homes of staff for individual specific or office specific items.

In the event of a call in or no show, Residential Team Leader should first try to find someone employed and experienced with the individual to cover the given shift. If no one can then Residential Team Leader will report to duty.

Grocery shopping will be completed weekly. It is the Residential Team Leaders responsibility to make sure that there are always adequate supplies in the house. This includes toiletries, cleaning supplies, food, drinks, and medical needs.

The Residential Team Leader will report corrective active needs to Program Director and Human Resources and Training Coordinator for investigation and appropriate discipline.  This is to include reporting of corrective actions for staff that are inappropriately conducting themselves (Including but not limited to: not following job descriptions, acting inappropriately at work, sleeping on the shift, failure to show up on a shift, etc.).  Complete staff supervision forms for all on-the job coaching provided to staff and submit to the Human Resources and Training Coordinator.

Respond to crisis situations as needed or instructed by the Residential Coordinator.

Create a staffing schedule for your assigned homes and ensure staff are available at all hour's individuals are in the homes.  Obtain approval for all overtime from the Residential Coordinator

Any other duties assigned to you by BluWell of Kentucky.  This includes following all directives and requests of the Executive Director and Residential Coordinator.
