What is an exempt H1B?

An Exempt H1B job refers to a position that is not subject to the annual H-1B visa cap because it is with a qualifying employer. These employers typically include institutions of higher education, nonprofit research organizations, or government research entities. Since they are not subject to the cap, they can sponsor H-1B employees at any time without waiting for the lottery process. This provides more flexibility for both employers and foreign workers seeking employment in specialized fields.

What skills and qualifications are needed for an exempt H1B?

An Exempt H1B professional is typically expected to have specialized knowledge or advanced degrees in their field, along with eligibility to work under the H1B program at exempt organizations such as universities or non-profits. Proficiency in industry-specific tools, compliance systems, or relevant software, as well as adherence to organizational and immigration guidelines, is important. Excellent communication, adaptability, and cultural competency help Exempt H1B professionals integrate well into diverse teams and collaborate effectively. These skills and qualifications ensure legal compliance, strong performance, and a positive work environment within organizations that sponsor H1B-exempt employees.

What types of organizations hire exempt H1B professionals, and how does this affect the work environment?

Exempt H1B professionals are most often employed by higher education institutions, non-profit research organizations, or governmental research entities that fall under the H1B cap exemption. These settings usually foster collaborative, mission-driven work environments, with a strong emphasis on teamwork, education, and research. As a result, employees often have opportunities for mentorship, interdisciplinary collaboration, and professional development. If you join one of these organizations, you can expect a supportive atmosphere and the possibility of career advancement within academia or public interest sectors.

How to find exempt H1B jobs?

To find exempt H1B jobs, focus on employers and roles that qualify for H1B cap-exempt status, such as institutions of higher education, nonprofit research organizations, and government research agencies. These employers do not count H1B visa caps and often require specialized skills, relevant certifications, or advanced degrees. Job seekers can search on employer websites, professional networks, and government resources for cap-exempt opportunities.

Why Join Us
Valley Baptist Physician Network offers a rare opportunity to be the region's only neurosurgeon at a DNV Comprehensive Stroke Center - the only one south of San Antonio - with immediate, built-in volume and a fully integrated neuro team behind you. You'll operate within a 586-bed ACS Verified Level II Trauma Center equipped with cutting-edge technology including Brainlab, AIRO, and da Vinci robotics, supported by a dedicated interdisciplinary team of endovascular neurologists, neurohospitalists, and physiatrists. This is a community that genuinely needs you, in a health system that has been the regional standard-bearer for nearly 100 years. If you're looking for clinical impact, financial upside, and long-term stability, the Rio Grande Valley delivers all three.
Position Highlights
  • Solo neurosurgeon with a fully built-out neuro infrastructure: two endovascular neurologists, two neurohospitalists, outpatient neurology (general, epilepsy, stroke), and two physiatrists
  • Approximately two clinic days per week, 15-20 patients per day
  • Surgery days as needed with block OR time available
  • Call: 10 days/month of paid primary call plus 10 backup call days (backup rarely activated)
  • Paid call at $2,800/day in addition to base salary and production bonuses
  • Dedicated Certified First Assists in the OR
  • Clinic located in the professional building directly attached to Valley Baptist Medical Center - Harlingen
  • State-of-the-art 14-bed Neuro ICU
  • Privileges at Valley Baptist - Harlingen with potential for additional privileges at sister facility in Brownsville
  • EMR: Cerner
  • Visa sponsorship available for J1 and H1B non-cap exempt candidates

What We Offer

  • Malpractice insurance provided
  • Health, dental, and life insurance
  • 401(k) with employer matching
  • Deferred compensation program
  • Relocation assistance

Candidate Requirements
  • M.D. or D.O. from an accredited medical school
  • Successful completion of an ACGME-accredited Neurosurgery residency
  • Board Certified or Board Eligible in Neurosurgery

Why Harlingen
Harlingen sits at the heart of the Rio Grande Valley, one of Texas's fastest-growing regions, where year-round semi-tropical weather, no state income tax, and an exceptionally low cost of living make for an enviable lifestyle. South Padre Island's beaches are just an hour away, and the area's family-friendly communities are served by strong local schools and a welcoming, tight-knit culture. Whether you're drawn to outdoor living, international dining, or proximity to the Texas-Mexico border culture, the Valley offers something most markets can't match: affordability without sacrifice. For physicians who want to build a life - not just a career - Harlingen is an easy choice.
