The Role

We are seeking an energetic, compassionate, and organized Personalized Living Manager to lead our personalized care services and resident-centered programs at Martha Franks!

In this key leadership role, you will oversee the delivery of individualized support services, manage direct care staff, and collaborate closely with residents and their families to ensure every individual receives care tailored to their unique needs and preferences. If you excel at team management, clinical coordination, and building meaningful relationships with seniors, we want to hear from you!

Key Responsibilities
  • Care Plan Coordination: Assess resident needs and develop, implement, and update individualized personalized living care plans to promote independence and dignity.

  • Team Leadership: Recruit, schedule, train, and supervise direct care staff and personal care aides, fostering a supportive, accountable team environment.

  • Resident & Family Advocacy: Serve as the primary point of contact for residents and family members regarding personal care services, addressing questions, and resolving concerns promptly.

  • Regulatory & Safety Compliance: Ensure all personalized care services adhere to state regulatory guidelines, infection control standards, and facility policies.

  • Interdisciplinary Collaboration: Work closely with Wellness, Dining, Maintenance, and Therapy teams to provide holistic, continuous support across campus.

Qualifications
  • Experience: Minimum 2–3 years of supervisor or manager experience in Senior Living, Assisted Living, Home Health, or Personal Care settings.

  • Education: High School Diploma or equivalent required; Associate’s or Bachelor’s degree in Healthcare Management, Human Services, or Nursing (LPN/RN) preferred.

  • Leadership Style: Strong organizational, communication, and interpersonal skills with an approachable, hands-on leadership style.
