The Barton House is a 20-bed memory care community dedicated exclusively to caring for individuals living with Alzheimer's disease and other forms of dementia. Our intimate, home-like setting allows us to provide highly personalized care in a safe and supportive environment. Residents enjoy private studio rooms, home-cooked meals, meaningful activities, and a compassionate team that treats every resident like family. Position Overview
The Licensed Practical Nurse (LPN) or Registered Nurse (RN) plays a key role in providing compassionate, high-quality clinical care to our residents. This position is responsible for medication administration, monitoring resident health, communicating with families and providers, and supporting the caregiving team. The LPN works closely with the Executive Director and leadership team to ensure residents receive safe, person-centered care in accordance with state regulations and community standards.
Key Responsibilities
  • Administer medications and treatments in accordance with physician orders and state regulations.
  • Monitor residents for changes in physical or cognitive condition and report concerns promptly.
  • Maintain accurate and timely clinical documentation.
  • Assist with resident assessments and care plan updates as needed.
  • Communicate effectively with families, physicians, pharmacies, and outside healthcare providers.
  • Support and guide caregivers and medication aides during shifts.
  • Respond to resident health concerns and emergencies appropriately.
  • Ensure compliance with assisted living and memory care regulations.
  • Participate in care conferences and family communication when needed.
  • Assist with training staff on proper care techniques and dementia care best practices.
  • Promote a positive, respectful, and team-oriented work environment.
Qualifications
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing.
  • Experience in geriatrics, assisted living, memory care, or long-term care preferred.
  • Knowledge of medication administration and documentation practices.
  • Strong clinical observation and communication skills.
  • Compassion, patience, and a genuine passion for caring for individuals living with dementia.
  • Ability to work collaboratively with caregivers, leadership, and families.
Working Environment
This role operates within a memory care residential setting and involves frequent interaction with residents, families, staff, and healthcare providers. Light lifting may occasionally be required.
